After you have made your appointment, you will receive an email to set up your patient portal. Once your account is activated, you will be able to complete the required questionnaires and consent forms. These forms MUST be completed at least 24 hours prior to your visit to allow adequate time for review and to confirm insurance eligibility.
Cancellations & No-Shows: At least 24 hours notice is required for cancellations or rescheduling. Failure to provide adequate notice may result in a $100 cancellation fee.
Late Arrivals: If you arrive late for a virtual appointment, the session may be shortened or rescheduled at the provider’s discretion.
