Welcome to Lori WHNP, LLC (“we,” “our,” or “us”). These Terms and Conditions govern your use of our website, telehealth services, and any other offerings provided by Lori WHNP, LLC. By accessing or using our services, you agree to abide by these terms.

2. Telehealth Services
• Our practice offers virtual healthcare services in accordance with state and federal regulations.
• You must be located in a state where our provider is licensed at the time of your appointment.
• By using our telehealth services, you consent to remote consultations and acknowledge the limitations of virtual care.
3. Appointment Policies
• Scheduling: Appointments must be scheduled in advance through our online booking system or the CharmHealth patient portal.
• Cancellations & No-Shows: We require at least 24 hours notice for cancellations or rescheduling. Failure to provide notice may result in a $100 cancellation fee.
• Late Arrivals: If you arrive late for a virtual appointment, the session may be shortened or rescheduled at the provider’s discretion.
4. Payment & Billing
• We are in-network with Blue Cross Blue Shield of Illinois PPO and BlueChoice.
• If you are using one of the above insurance plans, copay is due at the time of service & we will submit a claim to your insurance for the remaining balance. If you are self-pay, the cost of the visit ($259 for new patient visits, $129 for follow-up visits) is due at the time of service.
• Accepted payment methods include debit or credit card.
• You may request a superbill for potential reimbursement from your insurance provider.
5. Prescription Policy
• Medications may be prescribed when deemed appropriate by the provider.
• Prescription refills require an active provider-patient relationship and may necessitate a follow-up appointment.
6. Privacy & Confidentiality
• We comply with HIPAA regulations to protect your personal health information.
• By using our services, you acknowledge and consent to electronic communications, which may include email, patient portals, and telehealth platforms.

7. Use of Website & Content
